My form doesn’t look or function as I designed it. Why?

You may have copied the wrong code from the Publish page. There are two different HTML boxes: one for the one you have created, and one for a quick "Portable" version of the same form called Plain Portable HTML. The portable version is only used to collect basic information, and does not include any template that you may have selected. To get a version of your code that has the template you selected, choose either the HTML or JavaScript version of the code instead.
