Incident Report Template
Workplace injuries are few and far between if you're lucky, but every now and then one of your employees may get injured on the job, requiring that you fill out a report.
The Incident Report Template provides a dynamic form your organization can use to get information regarding any injuries. The form asks for details of the incident, such as whether assistance was provided to the patient and what kind (ambulance, police, fire engine, etc.)
