Many home health care services have team members that regularly facilitate home visits to check in on program participants. When completing home visits, it can be uncertain the potential safety hazards that may arise from the home environment. 

To ensure that team members are safe after completing home visits, they must reach out to their assigned partner. The team member completing the home visit must let them know that they have left the home of the participant. As well as confirming their safety so that the team is aware they do not need assistance.

This in turn allows staff to no longer need to spend valuable time tracking each other's schedules.  All while ensuring the risks of conducting home visits continue to have safety measures such as checking in and out.

Instead of a partner keeping track of when a team member should complete a home visit, this solution will do so. Alerts send out automatically when a team member does not check out of home visits after a certain amount of time. Allowing the team members partner to know if they need to reach out to confirm the team members safety.

Steps to Success

  1. Add each team member’s contact information to the solution to create a database for the team. This can occur through an online form, or by importing an Excel spreadsheet.
  2. Each time a team member arrives at a home visit, they will complete an online form with their name and location. This will create a timestamped record in the SimplyCast solution that shows when the team member began the visit.
  3. Once a team member completes a home visit and they have left the location of the visit, check out can occur. This is complete by doing another short online form which will confirm they have left safely. When check out is not complete after a specified time has gone by, they will automatically receive a reminder.
  4. Detailed reports generate at the end of the visit, allowing administration to review and make any adjustments.
